Fletcher Allen Health Care in Burlington, Vermont has an opening for a Tobacco Treatment Specialist Counselor

As a member of an interdisciplinary team which includes physicians, nurses, social workers and service coordinators, the Tobacco Treatment Specialist (TTS) Counselor is responsible for direct intervention with tobacco users in the in-patient setting.  Provides bedside cessation intervention and telephonic follow-up for in-patients identified as tobacco users and assists with smoke-free policy implementation.   The TTS Counselor utilizes motivational counseling skills and provides resource, referral, information and education to patients and staff.  The TTS Counselor serves as a primary liaison to the patient's primary care physician and to community-based resources to assist the patient in refraining from the use of tobacco products.  TTS is responsible for maintaining the referral system currently in place.

This position requires a lot of walking in a fast paced work environment. Must be able to work four days to complete consults prior to discharge.  Experience with EMR preferred.  Some data base management. Ability to educate health care professionals and work with multi-disciplianary team.  The ideal candidate will have experience in motivational interviewing and coaching people in lifestyle changes.

Education:

MSW or BSW preferred. Vermont RN or Respiratory Therapist from an accredited school considered.

Experience:

Minimum of three year experience and demonstrated competence in counseling techniques, substance abuse/additions and bedside education.

 MENTAL HEALTH / SUBSTANCE ABUSE CONSULTANT

Contribute to an exciting new initiative designed to increase access and outcomes for individuals served in primary care settings.  The Mental Health/Substance Abuse Consultant works through an interdisciplinary Community Health Team serving Northwestern Vermont.  Activities involve program development focusing on collaborating with primary care providers in screening for mental health and substance abuse, linking patients with necessary services, and providing short term solution focused care.  Seeking a full-time Therapist with program development experience who is a generalist and familiar with a range of mental health disorders, substance abuse, screening, brief treatment, and consultation.  The ideal candidate will be an MSW/LICSW.  Other licensures considered. 

 CLINICAL PROGRAM MANAGER

Program serving adults with severe & persistent mental illness, Community Rehabilitation & Treatment (CRT) is seeking a Clinical Program Manager to provide oversight of CRT leadership team and programs, including residential, employment, intensive case management, community support, and peer services.  Candidate will participate on division leadership team in coordinating and responding to unmet community needs.  Seeking someone with solid management & clinical skills who enjoys working in a fast paced community mental health setting.  The ideal candidate will hold a Master’s Degree and be licensed or license eligible, have strong program development skills and social policy experience, as well as, experience with evidence-based practices and services for individuals involved with the legal system. 

TEAM LEADER – ENHANCED RESIDENTIAL SERVICES

The Community Support and Rehabilitation Team (CRT) is seeking a highly organized and energetic individual to provide administrative and clinical oversight to the housing services and residential programs, serving individuals with severe and persistent mental illness.  Primary responsibilities include supervision of staff providing residential services, fiscal reporting, oversight of all residential services, participation in State and local initiatives and delivering services in a variety of community-based settings.  Ability to work in a fast-paced environment, openness to new program development and shared responsibility for CRT program intake and risk management.  Program development and supervision experience is preferred.  Strong writing and documentation skills, attention to detail, reliable transportation, clean driving record and good attitude are a must!  Masters Degree and Independent Licensure required. Ideal candidate will have a minimum of two years of relevant experience.  Schedule is M-F with minimum evening and weekend call.

 PEDIATRIC SOCIAL WORKER

 Seeking an individual to provide community and home based social work to children and their families.  This position includes diagnostic assessment, collaboration with medical professionals, and assessment of individual and family needs to improve social functioning.  Previous experience working with children and/or adolescents and their families is required. Candidate must possess strong collaborative and time management skills as well as an ability to work independently.  Only a Masters of Social Work is accepted along with 2 years of post graduate experience.

                                    Requires public speaking skills as well as basic research skills.

                                    Requires excellent written, verbal and interpersonal communication skills.

                                    Ability to work cooperatively and effectively with Chapter and National leadership staff and with external contacts.
